Making Health Care Better
A new pharmacy practice model took effect July 2012 – the following services are paid fully by the government when provided by pharmacists;
Provide qualified patients with the best medication plan to suit their needs (see plans below)
- Injections services (selected medication only)
- Provide prescription renewal (restrictions may apply based on Alberta Pharmacists Association Standards of Practice)
- Prescribe medication in emergency situations
What can a Comprehensive Annual Care Plan do for me?
Our pharmacists can assist you by:
* Providing you an assessment of your current medication and past medication history
* Confirming you understand how to use your medication
* Creating a care plan which includes:
a) Identification and resolution of existing or potential drug therapy problems
b) Goals of therapy that are mutually agreed upon between pharmacist and you
c) Identification of possible interventions to resolve drug therapy problems
d) Plans for follow up to address your drug therapy needs
Who’s eligible for the Comprehensive Annual Care Plan?
Albertans are eligible for this service if:
- They are a member of the Alberta Health Care Insurance Plan
- They meet directly with the pharmacist providing the service
Further, to be eligible for the service, a patient must have either:
- Two or more chronic conditions from group A (see list below)
- One chronic condition from Group A AND one or more risk factors from group B in order to meet the definition (see list below)
What can a Standard Medication Management Assessment Plan do for me?
Similar to the Comprehensive Plan, our pharmacists provide:
- Detailed assessment of your current medication and past medication history
- Confirmation that you understand how to use your medication
- An accurate plan for your medications, vitamins, herbal products and over the counter medications
- Notification to your family physician and other health care providers as appropriate, about the plan
- Documentation of the plan on your Records of Care
Who is eligible for the Standard Medication Management Assessment Plan?
Albertans are eligible for this service if:
- They are a member of the Alberta Health Care Insurance Plan
- They provide consent to participate
Further, to be eligible for the service, a patient must have either:
- Have at least one chronic medical condition from Group A and
- Be currently taking 4 or more different drugs or insulin (see pharmacist for details)
Group A (Chronic Diseases)
- Hypertensive Disease
- Diabetes Mellitus
- Chronic Obstructive Pulmonary Disease
- Asthma
- Heart Failure
- Ischaemic Heart Disease
- Mental Health
Group B (Risk Factors)
- Tobacco
- Obesity
- Addictions
