Please note the following:

• You will need to bring your health card to the appointment*
• When you arrive we will ask you to complete a screening form to ensure the vaccine is safe for you
• Wear clothing that allow easy access to your upper arm
• We recommend that you stay in the store for 15 minutes after your injection for observation
