Patients eligible for the Medication Review Program require a valid OHIP card, as well as if any of the following statements are true;

• You have been diagnosed with the following;  Type 1 or Type 2 diabetes.
• You reside in a licensed long-term care home.
• You are taking 3 or more chronic prescription medications.
